What we ask for
Enough to review your application — nothing more.
- Your name, contact details and location
- Your current role, organization, discipline and practice setting
- Your professional license type and issuing state, where you hold one (license number and NPI are optional)
- Your leadership responsibilities and professional interests
- Why you want to join and how you would like to take part
- Your agreement to our Membership Standards, Privacy Notice and Membership Terms
What we never ask for
No documents. No Social Security numbers.
We do not ask applicants to upload passports, identity documents or certificates, and we never ask for Social Security numbers. If staff need to clarify something, they ask a question — you answer in writing.
No payment is taken when you apply.
How review works
Every application is read by a person.
- Received. You receive a reference (NPHLA-2026-XXXXX) and a private status link by email.
- Under review. A member of the NPHLA membership team reviews your application. Where you provide an NPI, staff may check it against the public NPPES registry.
- Information requested (sometimes). If anything needs clarifying, we email you a question and you reply through your private link.
- Decision. We email you the outcome. Approved members receive account activation instructions, a Member ID and access to the NPHLA Member mark.
What reviewers consider
- That you work in, train for, or work closely with pharmacy and healthcare.
- That the membership category fits your role — reviewers may recommend a different category.
- That the information provided is complete and consistent.
- Your commitment to the Membership Standards & Code of Conduct.
What membership is — and is not
NPHLA membership is membership of a professional association. It is not a license, certification, credential or accreditation, and NPHLA does not verify that a member is licensed to practice. Always check licensure with the relevant state board.
You may withdraw your application at any time using your private status link. Questions about a decision? Contact the membership team quoting your reference.
